Application for Assistance

The information provided below describes how the Fuel Fund of Maryland (“FFM”) collects, uses, and discloses your information as a result of your participation in the Bill Assistance Program (“Program”).  To participate in the Program, you must consent to the information practices described below. 

Please read this page carefully and completely to understand FFM’s practices with respect to the information that it collects from or about you as a result of your participation in the Program.

Collection 

When you apply to participate in the Program, FFM will collect certain information about you, including your name, address, telephone number, email address, date of birth, utility account number, social security number, and other personal information. 

FFM will also request that your utility company provide FFM with information about your energy usage and bill payments pertaining to the 12 month period before and the 24 month period after the date of your application. 

FFM will also be provided with the information that you provided to Office of Home Energy Programs ("OHEP"). 

FFM will use this information to assist it in determining eligibility for FFM's energy benefits and programs. This information may include details about the amount of energy you use, new charges on your account, payments you make towards your account balance, your account balance, your connection status, and any disconnection notices you may receive. 

Use 

You understand that FFM may use the information that it collects from or about you, including energy usage and bill payment data provided by your utility company, and any information that you provide to FFM to conduct, evaluate, and report on the Program. 

FFM may also use this information for any purpose identified in the privacy policy on its website.

Disclosure

You authorize FFM to disclose your information to third parties, including your utility company, government agencies, consultants that assist us in evaluating your usage and payment data, and other entities that assist us in operating the Program.  FFM may also disclose this information for any purpose identified in the privacy policy posted on its website.

By clicking “I Agree” on the application, you represent that you have read and understand FFM’s information practices with respect to the Program. 

You further represent that you authorize FFM and its partners, including your utility company, to collect, use, and disclose information about you as provided above.
